I have one that does not stay tight after you drill a couple holes. Any tips here?

Why would you have the bit anywhere near the vice. The chuck is all you want to tighten. Last time I tried that on a fully charged cordless drill, it snapped the bit in half.
My "quick" reflex ain't what it used to be I guess..
