Back when there were lots of dairies around here I used to breed cows for ABS. Lots of times the dairymen would leave cows in the lot to breed. Many times the help would be inadequate observers/detectors and the cows would already be bred.

Question: How many times has an inexperienced AI person bred a previously bred cow and does it abort the cow every time?
Sometimes? Never? Always wondered the outcome.

I actually asked this same question in a dairy circle in college, my Professor said the likelyhood of it being aborted was pretty high for early pregnancys (especially considering the already have high embryonic death loss) the gun penetrating the pregnacy, uterine trauma, knocking embryo loose from uterine wall lots can go wrong but it doesn't always. It was estimated to me about 80-90% loss. I haven't ever seen research data on this though.
